Meaning:
Lit. “the operation was successful, the patient died”. This is an extremely sarcastic phrase you bring up if someone claims to have completed their task while not achieving their main goal. Or causing lots of damage that nullifies their apparent success. Somewhat related to “Pyrrhic victory”.
Sadly, the origin is the situation in which surgeons complete their tasks during a complex operation but somehow the patient they are trying to save still dies. This of course makes all their effort in vain and any claims to have been successful met by a sarcastic laugh or weeping from the relatives.
The phrase can be used in multiple contexts but you need to remember that by employing it you condemn someone, even if in an indirect and ironic way.

Operacja się udała – bo pod unijnym naciskiem Polska zaprzestała wycinki drzew na jej terenie. Pacjent zmarł – bo doprowadziło to do katastrofy ekologicznej.
